Where can I get a physical/etc. If your under 18, without parents knowing?

You can't get a physical without your parents knowing. Your parents have to sign papers if you're under 18. Some schools do physicals but you have to be signed up for a sport & you're parents have to sign papers. Just tell your parents you want/need a physical for school. Simple as that.
